Initial Assessment

The first step for anyone considering using HR&SS services is an initial assessment, which allows HR&SS to understand your situation and how we might be able to help. 

A qualified HR&SS worker will ask you some of questions about your current situation, such as the state of your health, where you are living, who cares for you, and how you prefer to communicate. Then they will talk to you about what you want to achieve and the kind of support you think would be useful. 

Once we have a clear picture of what you are trying to achieve, we will be able to suggest what opportunities and services are available to you. This may include services that we provide at HR&SS or it may be services from other organisations and businesses. If we suggest a different service for you, we will give you information about the different providers and help you to choose one that matches your needs and personality.  

We provide an initial assessment free of charge to all consumers, even if you choose not to use any of HR&SS’ services. For consumers who are returning to HR&SS or who have been assessed but feel their needs or circumstances have changed, we can provide a Review. 

  

Review

If you feel your needs or circumstances have changed since your initial assessment but you are not sure what this means for the services you need, HR&SS workers can conduct a review for you. This follows the same format as the Initial Assessment <link to Initial Assessment page> - our worker will ask you some questions about your life, your health and your goals in order to understand your situation. They will then discuss options for meeting your different needs and, if needed, find organisations that can help you. 

We recommend that you review your plan once year plus whenever you have a major life change. For example, it is a good idea to review your plan if you finish your studies (at any level), get married or divorced, have a child, move out of home or move interstate etc From time to time, you may even just change how you feel about life or your future – reviewing your plan at these times can help make sure that your services keep meeting your needs.
