Support Coordination 

We understand that you have better things to do with your time than call around care agencies to find a fill-in carer or sort through invoices from various specialists. To free up your time, our Service and Support Coordinators can take care of the running around for you. Additionally, we maintain quality assurance measures to ensure the providers you are using meet certain quality criteria. It’s like having your own personal assistant!

If you have a complex roster of carers, regularly change your care shifts, or just want the reassurance of having someone who can quickly organise a suitable replacement if your usual carer cancels, our Support Coordination could be just what you need.

Our Service and Support Coordination team maintains a list of suitable agencies and monitors their service to ensure that the agency is reliable, carers are appropriately qualified and meet your language and care requirements.

Service Coordination

Our Service and Support Coordinators can also manage the purchase and payment of various services you need including:

• professional cleaning
• computer and garden maintenance
• taxi vouchers
• travel and accommodation 
• physiotherapy, massage and other allied health services
• white goods (such as washing machines and refrigerators)
• health-related equipment

and other goods and services you need to live the life you want.

Case Management

Sometimes you might need a little extra help and support. That’s where Case Management comes in. Whether you are just finding life a bit of a struggle at the moment and need support to get back on track, or you have a particular goal you are having trouble achieving on your own, a Case Manager can help.

Case Managers can help with things like:

• getting ready to find a job – or getting started in your new job!
• getting  through a major life change, such as divorce or loss
• getting your first pet (how to choose the right pet, what you need to know to care for your pet etc) 
• re-locating to a new suburb or city 

An HR&SS Case Manager can help you to locate the tools and resources you need, be a sounding board for your ideas, guide your problem solving and generally encourage and support you. 

HR&SS also provides specialist Housing Case Management to help you to find and apply for modified and affordable homes.
